Applications are accepted on an ongoing basis. Completed applications may be submitted to the attention of:

Applications will be reviewed on a monthly basis. Applicants will be notified, in writing, as to the decision of the Foundation review committee within 90 days of proposal submission. Applicants should complete the attached cover sheet along with:

I. Description of applying organization, including: (2 page maximum)
a. history and mission
b. population served by organization
c. number of paid full-time and part-time staff;
number of volunteers
d. Information on organizations collaborating
on project (if applicable)

II. Proposal Summary:
(one-half page, maximum.)
Briefly explain why your agency is requesting
this grant and outcomes you hope to achieve.

III. Funding Request (3 page maximum)
1) Describe the program for which you are seeking funding. If application is for event sponsorship support, you may attach an event description page, targeted audience, plan to promote event and a sponsor benefit page.
2) Describe the population you plan to serve and how this population will benefit from your project. Include client profiles in this section.
3) Describe the strategies that you will employ to implement your project.
4) Anticipated length of the project.
5) Future plan for sustaining the project.

IV. Evaluation
Please explain how you will measure the effectiveness of your project. Please describe the results you expect to achieve by the end of the funding period.

V. Budget for proposed project including anticipated revenue and expenses.

VI. Attachments
1) A list of the Board of Directors
2) A copy of the IRS letter indicating your agency’s tax-exempt status under Section 501(c)(3).
